Cited medical record review
Medrecords AI is a software application designed for medical record review, chronology development, and report drafting. The tool processes unstructured medical records, including PDFs, scans, faxes, printed and handwritten notes, voice recordings, and DICOM imaging files, and generates structured outputs with citations tied to specific pages or timestamps. The platform is priced per unique page rather than per case, with rates starting at 10 cents per page and decreasing to 5 cents at higher volumes. No seat fees or subscriptions are required, and the first case is free. The software identifies duplicate pages in submitted productions so that users pay only for unique content. The application includes features for building medical chronologies indexed by date of service, flagging records that are referenced in the file but not provided, and answering questions about the record in three modes: clinical, evidence-based, and verbatim. It supports report drafting across 28 template types, with every line cited to its source. The platform also integrates DICOM viewing capabilities, including a PACS viewer and 3D rendering, allowing review of imaging studies alongside radiologist reports. Additional functions include detection of treatment gaps, duplicate billing, and unsupported charges, as well as logging of PHI access for audit purposes. Typical use cases include independent medical examination (IME) report preparation, causation and apportionment analysis, claims review, and damages assessment. The workflow involves uploading records, reviewing the auto-generated chronology and citations, answering case-specific questions, and drafting reports on the user's own letterhead and template. Physicians verify and sign the drafted reports. The platform is used by approximately 8,012 IME physicians and is positioned for legal and insurance contexts where defensible, source-traceable documentation is required.
